As a human milk donor, I am donating my breast milk to be used in accordance with the regulations relating to Human Milk Banks as set out by the Human Milk Banking Association of South Africa (HMBASA).

That the expressed donated milk will be processed and microbiologically tested to ensure its safe use for vulnerable infants, and at the discretion of the Ncelisa Human Milk Bank.

That I do not expect any portion of the expressed donated milk to be returned to me;

That I will not receive any compensation and/or derive any financial or other benefit for donating my excess breast milk;

That my identity as a donor of breast milk shall remain protected in that the donated milk shall be distributed without disclosing my personal detail in any way whatsoever;

That all test results shall be kept confidential.

That I will not incur any costs for donating my excess breast milk.

I undertake to inform the human milk bank of any changes in my health as soon as I become aware of it.

I confirm that I am 18 years of age or older.

I acknowledge that Ncelisa Milk bank may process my personal information in accordance with the Protection of Personal Information Act No.4 of 2013 (“POPIA”) and Netcare’s privacy policy which can be accessed on www.netcare.co.za/privacy

Participation in the anonymous satisfaction survey that will be e-mailed/sms’d to me is at my own discretion.
